Dr. Suba is a Nurse Practitioner specializing in Adult/Gerontology care practice. I work at Weill Cornell Medicine/NewYork-Presbyterian in the Division of Geriatrics and Palliative Medicine. My expertise revolves around geriatric care, complex care population management, and transitions of care. I am also well-versed in risk adjustment and ICD-10 coding and documentation.

Purpose
The presentation aims to communicate the effectiveness of the Falls Management Program on the knowledge of all facility staff regarding falls prevention among older adult (OA) residents in a resource-constrained long-term care facility (LTCF). It also aims to publicize the effectiveness of the program in improving fall outcomes in the LTCF. The Falls Management Program is a quality improvement (QI) initiative grounded on strong research evidence, which is specifically designed to manage fall care processes and outcomes in long-term care practice settings. For this reason, the presentation relates well to the theme and objectives of the conference.
